Just fill the application out and mail it with your paystubs.



1) get the application and fill it out:

2) get paystubs for the month before you applied.

mail to famis (the link above has more detailed instructions)



if you go through a case worker, it takes longer!!!! so mailing is the fastest way (many times only 2 weeks)

You get a blue and white card for dental.
