| Adult Day Services | | Circle Center services and programs are planned to meet the needs of older adults and their caregivers: Individualized, multidisciplinary care plan (shared with the family and MD) that includes cognitive, emotional, physical, and health-related needs and services. Individual and group therapeutic activities that support cognitive and physical functioning and promote social interaction. Personal care assistance. Daily nursing care and supervision including medication administration. Evaluation of activities of daily living at intake, and continuous monitoring and assistance. Counseling regarding personal adjustments and life changes.Nutrition (breakfast, lunch and two snacks), including special diets, tube feedings, and ongoing assessment to determine changing needs. A Montessori-based activity program for participants with dementia, including a Snoezelen room. An alarm system on all doors, an enclosed patio for safety, and a sprinkler system throughout the Center.
